Separation of Employment

COBRA Continuation

Your core benefits (medical, dental, and vision) end on the last day of the month your employment ends. All other benefits end on your last day of employment. When your benefits end, you may choose to continue medical, dental, vision, and flex spending accounts under COBRA continuation. Information on extending these benefits will be mailed to you.

COBRA continuation is administered by UnitedHealthcare. For questions regarding enrollment or payment, please call 866-747-0048.

Life Insurance Continuation Coverage

Basic and Supplemental Life Insurance continues through your last day of full-time employment. You may elect to continue Basic and/or Supplemental Life Insurance by porting coverage to a Group Term Life Plan or converting the coverage to an Individual Whole Life Plan. Access to UKG

Upon separation of employment you will have access to UKG through May 31 of the year following your final payment. For example, if your last check was paid in 2020, you will have until the end of May 2021 to access UKG.

Important

Life events must be submitted within 30 days of the event, or within 60 days for a birth, adoption, placement for adoption, or changes to Medicaid or SCHIP eligibility.
